[Pancitopenia induced by vancomicina: case report].

Introducción La vancomicina es un antibiótico glicopéptido tricíclico con efecto bactericida, que inhibe la formación de peptidoglucano, bloqueando la síntesis de la pared celular bacteriana. Tiene actividad frente a bacterias Gram positivas y es especialmente utilizado en infecciones causadas por Staphylococcus aureus meticilin-resistente (SAMR), como bacteriemias, endocarditis, neumonías, osteomielitis e infecciones de partes blandas. Entre los efectos adversos asociados a la vancomicina más importantes destacan: nefrotoxicidad, ototoxicidad, flebitis, fiebre, hipotensión, eritema y “síndrome del hombre rojo”, caracterizado por un enrojecimiento de la parte superior del tronco y cuello, relacionado con una rápida velocidad de infusión, causando una reacción de tipo histaminérgico1. También han sido descritas reacciones de hipersensibilidad graves como síndrome Steven-Johnson, síndrome de DRESS o necrosis epidérmica tóxica. A nivel hematológico, se han observado casos de neutropenia y trombocitopenia. En el caso de la neutropenia, ha sido vinculada a un uso prolongado, con una frecuencia que puede oscilar entre 2-12%2,3. El caso de la pancitopenia inducida por vancomicina es una reacción más excepcional, con muy pocos casos publicados en la literatura4-7. A continuación, presentamos un caso de pancitopenia reversible secundaria a la administración de vancomicina.
